Care Manager

Responsibilities

• Coordinate patient care across the continuum to ensure safe transitions and appropriate resource utilization.
• Conduct clinical and psychosocial assessments to develop individualized care plans.
• Collaborate with physicians, nurses, social workers, and community partners to optimize patient outcomes.
• Educate patients and families on care processes and post-acute services.
• Address barriers to care and support complex discharge planning.
• Document thoroughly in the EMR, maintaining a patient-centered approach.
