Responsibilities
- Coordinate and oversee resident care plans and services
- Communicate with residents, families, healthcare providers, and staff
- Monitor residents’ needs and ensure proper follow-up
- Assist with admissions, assessments, and ongoing resident support
- Maintain accurate documentation and records
- Cookordinate medical appointments, services, and outside providers
- Help ensure compliance with facility policies and regulatory requirements
- Work closely with administration and department leaders to support daily operations
Qualifications
- Experience working with seniors, adult homes, assisted living, healthcare, or case management preferred
- Strong organizational and problem-solving skills
- Excellent communication and interpersonal abilities
- Ability to multitask in a busy environment
- Caring, patient, and professional attitude
- Computer and documentation skills required
Pay
$25-$28 per hour (based on experience)
