Care Manager

About the role

The Caregiver provides hands-on support to residents, helping with activities of daily living (ADLs) such as bathing, dressing, grooming, mobility, and personal care, while promoting safety, dignity, and independence.

Responsibilities

• Follow Individualized Service Plans (ISP) and document changes accurately
• Engage residents through conversation and daily activities, fostering meaningful relationships
• Support dining services and maintain a clean, welcoming environment
• Communicate effectively with families and care team members
• In Memory Care, create enriching experiences for residents with memory loss

Qualifications and Skills

• High School Diploma or GED; age 18+
• CPR/First Aid certification or ability to obtain
• Compassion, reliability, and strong communication skills
