Responsibilities
- Provide intensive case management services to individuals with complex needs.
- Ensure clients have access to mental health, medical, and community resources that support community integration.
- Cookordinate care plans and adjust them according to the evolving needs and goals of each participant.
- Facilitate smooth transitions by collaborating closely with hospital discharge teams, clinical staff, and community service providers.
- Conduct field-based outreach and follow-up, ensuring continuity and consistency of care.
- Support and empower clients to navigate systems independently and advocate for their needs.
Qualifications and Attributes
- Strong organizational and time management skills.
- Effective problem-solving and critical thinking abilities.
- Capacity to manage and prioritize complex caseloads.
- Demonstrated ability to work across systems (healthcare, housing, disability services).
- Empathetic, client-centered approach with strong interpersonal communication skills.
- Spanish language proficiency is a plus.
Qualifications
- Bachelor’s degree in social work, psychology, or a related field is required.
- Minimum of one year of experience in a care management or case management setting.
- Experience working with individuals with mental health needs and/or intellectual and developmental disabilities (I/DD) strongly preferred.
Physical Characteristics
- Must be capable to access all rooms in a 2-story home.
- Able to work in an open space floor plan.
- Must be capable to move throughout the work day.
- Occasional lifting of >_25+ pounds.
