About the role

The Care Manager Assistant - LPN supports patient care coordination by monitoring, evaluating, and promoting quality services in compliance with laws and standards.

Qualifications

• Nursing diploma (associate’s or bachelor’s degree)
• Active LPN license
• Preferred clinical experience in post-acute care, case management, utilization review, or discharge planning

Responsibilities

• Ensure clear communication and organizational efficiency
• Exercise sound decision-making and adhere to policies
• Maintain confidentiality of patient information
• Utilize computer systems for care coordination
• Liaise with external contacts (e.g., healthcare providers, families)
• Train staff and families on care standards and procedures
• Implement and uphold care standards
• Deliver excellent customer service
• Assist in patient evacuation during emergencies
• Function effectively within a team environment
