About the Role

The Care Manager Assistant - LPN supports patient care coordination by promoting quality services through a collaborative process that monitors and evaluates care plans, ensuring compliance with laws and standards.

Qualifications

• Active nursing diploma (associate or bachelor’s degree)
• Valid LPN license in good standing
• Preferred clinical experience in post-acute care, case management, utilization review, or discharge planning

Responsibilities

• Generate clear, organized written communication and prioritize tasks effectively
• Support organizational goals, implement policies, and adhere to legal and procedural guidelines
• Maintain professional relationships, confidentiality, and uphold integrity and compliance standards
• Engage in patient, family, and community education; serve as liaison with external providers
• Utilize computer systems for data entry and retrieval, and demonstrate excellent customer service
• Assist with patient safety, evacuation procedures, and interdisciplinary team collaboration
