Care Management Specialist PRN

About the role

The Care Management Specialist PRN will provide both weekday and weekend coverage. They will work closely with the Care Management Team to provide direct patient care and assist in the coordination of team-delivered services.

Responsibilities

• Complete psychosocial screenings and more thorough assessments as needed to contribute to the patient’s individualized transdisciplinary treatment plans.
• Offer psychosocial support and intervention.
• Educate patients, their family members, team members, and social work students.
• Facilitate compliance with treatment.
• Work with the community to provide needed services.
• Coordinate discharge planning activities.
• Work directly with patients, families, the rehab team, third party payers, and external agencies to develop a coordinated rehabilitation program.
• Serve as a resource to other team members regarding prudent utilization practices and payer requirements.

Requirements

• Previous experience in a healthcare setting providing clinical social work and/or case management services required.
• Acute care or inpatient rehab experience preferred.
• Experience in an academic healthcare setting and/or clinical experience with specialty patient populations (Traumatic Brain Injury, Spinal Cord Injury, and Stroke) also preferred.

Qualifications

• Master's Degree of Social Work from a Council on Social Work Education accredited program or Baccalaureate Degree in Nursing from an accredited School of Nursing preferred.
• Registered Nurse in Virginia or License required if a licensed registered nurse.
• Licensed Clinical Social Worker preferred.
• Certified Case Manager certification preferred.

Skills

• Strong interpersonal and communication skills.
• Ability to work independently and as part of a team.
• Knowledge of rehabilitation principles and practices.
• Proficiency in Microsoft Office Suite.
