Care Management Social Worker, Part-Time
About the role
The role involves providing grief counseling, disease adjustment support, crisis intervention, goals of care planning support, and de-escalation services for patients. The primary focus is on assessing patients and families' wholistically for discharge planning needs.
Responsibilities
- Assesses patients and families for discharge planning needs in various settings (inpatient, observation, and emergency departments).
- Reviews medical records and incorporates clinical, social, and financial factors into the transition of care plan.
- Develops discharge plans with contingency plans to ensure timely care coordination and progression of care.
- Leverages technology and follows standard work and best practices to communicate with post-acute care services and facilities.
- Participates in multi-disciplinary rounds to review patient status, progression, and discharge plans.
- Communicates with and educates patients and families regarding emotional, social, and financial impacts of illness.
- Facilitates patient and family care conferences with the multidisciplinary team.
- Documents discharge planning evaluations, ongoing assessments, discharge plans, MDRs, barriers to progression of care, avoidable days, and patient and family needs.
- Provides patient and family advocacy and supports patient's choice and rights during hospitalization.
- Communicates with payors to obtain authorization for post-acute care as needed.
- Assesses readmitted patients for their perceived reasons for readmission.
Requirements
Requires 2+ years of care management experience and a Master's degree in a related field. A Master's degree in Social Work is also required. Additional certifications such as Accredited Case Manager (ACM) or Certified Case Manager (CCM) are preferred.
Qualifications
Excellent interpersonal communication and negotiation skills, critical thinking and problem-solving skills, psychosocial assessment skills, customer service skills, effective organizational skills, computer proficiency with Outlook e-mail and electronic medical records, flexibility, adaptability, understanding of pre-acute and post-acute venues of care, and knowledge of state and federal guidelines pertinent to care management are required.

Pay
$24.01 - $44.65 per hour
Schedule
Part-time, 8:00am-4:30pm, rotating Thurs/Fri and Thurs/Fri/Sat shifts.
