About the role
As the Care Management Services Registered Nurse, you will provide care management and coordination for patients enrolled in the Care Management Program within the Primary Care setting. You will collaborate with primary care teams, specialists, patients, families, and caregivers to deliver quality, efficient, and cost-effective healthcare services.
Responsibilities
- Provide care management and coordination for patients in the Care Management Program.
- Collaborate with primary care practices, specialists, and interdisciplinary healthcare teams.
- Ensure delivery of quality, efficient, and cost-effective healthcare services.
- Engage with patients, families, and caregivers to address medical and social needs.
- Conduct site visits as required, utilizing an automobile with valid Hawai‘i auto insurance.
Requirements
- Current Hawai‘i RN license.
- Current American Heart Association Health Care Provider Basic Life Support (BLS) CPR card.
- Current driver’s license and valid Hawai‘i auto insurance with access to an automobile for site visits.
- Two (2) years of clinic experience in a Primary Care setting.
- Experience working within a collaborative interdisciplinary team.
Preferred Qualifications
- Certification in Case Management.
- Experience in care coordination for patients.
- Care management experience.
- Experience participating in continuous quality improvement.
- Working knowledge of Electronic Medical Records (EMR).
Pay
125,174 – 156,478 USD per year. Rate of pay is determined by knowledge, skills, abilities, relevant experience, training, and internal peer equity.
Schedule
- Full Time Regular (1.0 FTE).
- Day shift – 8 hours.
