PeaceHealth is seeking an experienced RN Care Manager to join our collaborative team and play a vital role in helping patients navigate their healthcare journey. Responsibilities
- Coordinate and facilitate individualized discharge plans across the continuum of care
- Assess patients' clinical, psychosocial, environmental, and financial needs
- Collaborate with physicians, nursing staff, therapists, and community resources to ensure smooth transitions of care
- Identify barriers to discharge and proactively develop solutions
- Conduct utilization reviews to ensure appropriate level-of-care determinations
- Partner with payors and patient financial services to support authorization and reimbursement processes
- Facilitate interdisciplinary care team meetings focused on quality, efficiency, and patient satisfaction
- Support patients and families through complex healthcare decisions and care planning
- Maintain accurate and timely documentation in accordance with regulatory and organizational requirements
Requirements
- Graduate of an accredited School of Nursing
- Current Registered Nurse (RN) license in the state of practice
- Basic Life Support (BLS) certification upon hire
- Minimum three years of clinical nursing experience in an acute care hospital setting
Equivalent experience in skilled nursing, outpatient case management, home health, or hospice may also be considered.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN)
- Master of Science in Nursing (MSN)
- Utilization Management or Quality Management experience
- Specialty certification
- Knowledge of third-party reimbursement and healthcare regulations
