Care Management Representative(RN) - SHMG

Responsibilities
- Participates in the referral process by assessing patient/family needs, communicating needs to multidisciplinary team, and reporting recommendations back to referral source.
- Advocates for patient/family within the health care system and community.
- Assesses patients for the need for medical care coordination - consulting physician as necessary.
- Utilizes case management process by establishing and implementing a plan to proactively facilitate achievement of outcomes through communication with patient/family and health care team to coordinate care.
- Focuses coordination of care on maximizing a patient/family's available resources (both financial and social/emotional).
- Communicates with the Clinical Team on a regular basis as to the status of patients under case management.
Requirements
- Associate’s degree in nursing, BSN preferred.
- Experience in case management is strongly desired.
- Requires a working knowledge of families within community systems and application to case management process.
- Certification as a Case Manager desired.
Qualifications
Strong interpersonal skills, excellent communication abilities, and a collaborative mindset are essential. Knowledge of community systems and the ability to advocate effectively for patients and families are also required.

Pay
$35.13 - $56.88/hr based on experience and qualifications.
