  • Develops and delivers educational materials and training programs for Care Management.
  • Collaborates with Care Management leadership to identify educational gaps and create campus-specific educational plans.
  • Conducts New Hire Care Management training to ensure new care managers receive necessary education and tools.
  • Mentors and trains care management team members to enhance workflow processes and service quality.
  • Supports performance improvement activities through problem identification, assessment, data gathering, change implementation, and evaluation of outcomes.
  • Participates in Process Improvement Initiatives and ensures practice changes are implemented.
  • Audits care management compliance with regulatory regulations and documentation standards.
  • Reviews and revises Care Management Policies, SOPs, and Job Descriptions as needed.
  • Identifies community resources and coordinates education opportunities for Care Management staff.
