About the role
The Care Management Coordinator provides support to the pharmacist-led chronic disease management program and assists with chronic disease management and medication therapy management. The Coordinator reviews registry data and other data sources to determine, based on guidelines, which patients are eligible for chronic disease management and performs outreach to enroll appropriate patients in the program.
Requirements
- High School diploma
- Pharmacy technician certification and 2 years of clinical experience preferred
- Bilingual English/Spanish preferred
Pay
$25.46 - $31.83 hourly
Actual salary offers are considered by various factors, including budget, experience, skills, education, licensure and certifications, and other business considerations. The range is subject to change. This role may also qualify for discretionary bonuses or incentives.
