Care Management Associate (Non-Exempt)
About the Role
In collaboration with the interdisciplinary team, Care Manager Associates support the Care Management team in effective transition planning and length-of-stay management. The role focuses on coordination of information, communication, and clerical functions to ensure patients receive the right information at the right time. This position offers an opportunity for an independent, motivated individual interested in collaborating with healthcare team members.
Responsibilities
- Communicates patient/family concerns or issues with Care Managers and Social Workers to ensure continuing care needs are addressed.
- Provides assistance and support for the Care Management team, including delivering and retrieving prescriptions to Mercy Tower Retail Pharmacy.
- Obtains quotes from transport companies for patient transportation from Mercy to post-acute destinations.
- Delivers patient choice letters and facility lists to patients and families.
- Prints and distributes Important Message from Medicare (IMM) letters to patients and documents appropriately in Epic EMR.
- Checks for consult orders in Epic EMR and informs Care Manager/Social Worker in a timely manner.
- Uses creative thinking to identify and resolve problems, actively participating in developing solutions to improve efficiency, effectiveness, and resource utilization.
- Works on performance improvement plans to increase coworker and patient satisfaction.
- Identifies potential risk management or clinical quality/safety issues and resolves them through intervention or collaboration with the healthcare team.
- Observes standard precautions and wears proper personal protective equipment (PPE) when appropriate.
- Maintains knowledge of and adheres to Mercy facility and Care Management department policies and procedures.
- Actively participates in ensuring compliance with accreditation agency standards, state, and federal regulations.
- Resolves situations within the scope of the job and handles complaints/concerns promptly and courteously.
- Keeps leadership informed of problem situations and actively participates in coworker meetings and surveys.
- Develops professional relationships with hospital and community partners while representing Mercy Care Management professionally.
- Demonstrates ethical behavior, maintains confidentiality, and adheres to HIPAA policies.
- Acts as a role model, values diversity, and treats all individuals with dignity and respect.
- Attends and participates in Mercy committees as assigned and maintains a professional appearance and workspace.
- Meets Mercy facility attendance standards and demonstrates Mercy’s mission, values, and Charism.
Requirements
- High School Diploma (minimum required).
- 1 year of healthcare experience preferred (may include healthcare student experience). Recent hospital experience is preferred.
- BLS (CPR) certification required at hire or within 90 days of hire.
- Excellent interpersonal and organizational skills.
- Ability to work with and preserve confidential information.
- Ability to deal effectively with diverse groups of people.
- Ability to work independently and in a self-directed manner.
- Strong verbal, written communication, and facilitation skills.
- Strong time management, prioritization, and critical thinking skills.
- Knowledge of Epic preferred.
Working Conditions

The position involves balancing multiple tasks, prioritizing effectively, and concentrating in areas of high activity. Work settings include cubicles, nursing units, or other healthcare environments.
- Equipment used: routine office equipment, laptop or desktop computer, copy machines, fax machines, telephones, and cell phones.
- Frequent movement, walking, and relocation required.
