About the role
This position supports the Care Management department at Regions Hospital, providing administrative support to ensure timely delivery of important patient communication and notifications. The main focus is to assist the care management team (social workers and care managers) with discharge planning.
Responsibilities
- Supports the Care Management team with administrative tasks, including patient chart auditing and data collection/review related to patient care outcomes.
- Affords assistance in discharge planning for the care management team.
Requirements
- Associate’s Degree in relevant field or equivalent experience.
- 1-2 years of direct patient care experience as a Nursing Assistant, Medical Assistant, Health Unit Coordinator, Patient Care Tech, etc.
Preferred Qualifications
- Bachelor’s Degree in relevant field or equivalent experience.
- 3 years of direct patient care experience as a Nursing Assistant, Medical Assistant, Health Unit Coordinator, Patient Care Tech, etc.
- Certified Nursing Assistant (CNA) or Health Unit Coordinator training.

Pay
Compensation is commensurate with experience.
Schedule
The position is full-time, Monday through Friday, with no weekends or holidays.
