Care Coordinator- YCOT

Brief Description

Responsibilities

• Performing community-based crisis assessments to determine needed level of care.
• Providing crisis intervention to individuals to reduce symptoms, provide education and prevent admission to a more restrictive environment.
• Collaboratively creating a treatment plan with the consumer to outline treatment goals.
• Providing case management activities to assist consumers in gaining and coordinating access to necessary care and services in accordance with treatment plan goals.
• Maintaining consumer progress with treatment goals and linkage to services.
• Informing family members about the individual’s mental health symptoms, treatment, and assisting with referrals to community resources.
• Completing electronic documentation each work day as outlined in YCOT Operating Guidelines.
• Following the key principles of a trauma-informed approach in all interactions.
• Performing related work as assigned and adapting to changing work requirements.
• Demonstrating ethical conduct in all professional activities.
• Complying with Agency policies and procedures and YCOT Operating Guidelines.
• Working any assigned crisis days.

Requirements

• Maintain a valid Texas Driver's License and be insurable under Agency policy.
• The ideal candidate will be required to transport consumers using their personal vehicle and will be reimbursed for mileage by the Agency.

Qualifications

• Bachelor’s degree (B.A.) from four-year accredited college or university with a major in a social, behavioral, or human service field, including but not limited to: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, and educational psychology, required.
• Minimum of one year experience in the mental health field, preferred.

Other Key Qualifications

• Maintain a valid Texas Driver's License and be insurable under Agency policy.
• Must be able to work autonomously in a fast-paced environment.
