About the role

Kitsap Mental Health Services is seeking a Care Coordinator to join the WISe Team providing comprehensive care coordination services for youth enrolled in the WISe program. This role involves direct service delivery, team facilitation, resource navigation, and crisis intervention.

Responsibilities

  • Provide care coordination services, including individual, group, and family counseling, crisis intervention, and case management.
  • Facilitate Child and Family Team (CFT) meetings to ensure family voice and choice are central in the wraparound process.
  • Assist families in navigating resources, including mental health services, education, legal, and social service systems.
  • Develop and update individualized Plans of Care, ensuring alignment with treatment goals and client needs.
  • Conduct CANS screenings and assessments, updating them every three months to guide service planning.
  • Collaborate with schools, courts, and social services to support youth and families effectively.
  • Advocate for youth and families, ensuring culturally competent and family-centered care.
  • Provide crisis intervention and coordinate with Designated Crisis Responders as necessary.
  • Document client progress accurately and timely, maintaining compliance with agency policies and procedures.
  • Participate in daily organizational meetings and treatment planning reviews.
  • Educate families on substance use issues and coordinate with internal and external providers as needed.
  • Monitor youth under Least Restrictive Alternative (LRA) court orders, facilitating extensions and ensuring compliance.

Requirements

  • Education: Bachelor’s Degree in Psychology, social services, or behavioral health field.
  • Experience: Entry level (no prior related work experience).
  • Driving: Required to meet agency driving requirements, including being at least 21 years old, having been licensed for a minimum of three years, successfully completing a motor vehicle history check, possessing and maintaining a current, valid driver’s license in the state of Washington, and having reliable, insured transportation.
  • Licensure: Agency Affiliated Counselor Registration.

Preferred Qualifications

  • Experience: Experienced (minimum 2 years of job-related experience).
  • Experience in co-occurring disorders treatment, individual and group therapy, vocational services, or substance abuse treatment.
  • Capacity to work well with children and families.

Skills

  • Establish and maintain effective working relationships with other employees, clients, organizations, and the public.
  • Communicate clearly and concisely.
  • Operate a personal computer, fax machine, printer, and copier proficiently.

Knowledge

  • Current trends in community mental health, including health care reform and automation of job tasks using computer technology.
  • Child and Adolescent Needs and Strengths Assessor certification and ability to administer this screening in person and over the phone.
