About the Role

Gaudenzia's Mission is to promote long-term recovery by providing comprehensive treatment and support to people with substance use disorder, co-occurring mental health disorders, and related conditions, regardless of individual circumstances.

Qualifications

Education: A minimum of an Associate's Degree (Bachelor's preferred) in a Human Service field or full certification as an Addictions Counselor

• Associate's Degree: Five (5) years of experience
• Bachelor's Degree: Three (3) years of experience
• Master's Degree: Two (2) years of experience
• Certified Addictions Counselor (CAC): Five (5) years of experience

Responsibilities

• Provide case management services, discharge planning, and care coordination for individuals transitioning from an American Society of Addiction Medicine (ASAM) Level Residential Substance Use Disorder Treatment (SUD) Program (3.7, 3.5, 3.3, 3.1)
• Handle a caseload of clients to aid them in achieving adjustments to specific barriers and situations of a difficult or complex nature by connecting them with the appropriate community resources
• Duties may include, but are not limited to, providing intake services to clients; preparing reports, treatment plans, discharge/continuing care plans, and keeping necessary records within client database and electronic health records
• Coordinate the activities of one or more groups of clients
• Generate referrals and aftercare plans
• Work in accordance with established regulations, policies, and procedures; exercise considerable initiative and independent judgment in achieving objectives
• Work reviewed by the Program Director, Program Supervisor, or Clinical Supervisor through the completion of assignments
