Responsibilities

• Lead continuity and consistency of care across the continuum (inpatient, emergency, and ambulatory/outpatient) to ensure integrated delivery, including comprehensive discharge planning and follow-up care.
• Develop effective working relationships with the Care Management Team to engage patients/families, collaborate, advocate, and problem-solve to support functional ability and ensure timely discharge plans.
• Monitor daily progress toward discharge plans, prioritizing high-risk patients for complications, admissions, or readmissions.
• Educate patients and families with chronic illnesses on evidence-based care standards, including self-management strategies.
• Identify support needs for patients and families, develop action plans, and guide self-management strategies.
• Educate patients and families about the healthcare system and facilitate relationship-building across care settings.
• Ensure patients have access to prescriptions, durable medical equipment (DME), and other identified services.
• Contribute to team problem-solving through communication, collaboration, data collection, and evaluating treatment outcomes.
• Track patient progress toward care plan goals and revise plans as needed.
• Advocate for patients to optimize healthcare needs, including safety, physical, legal, and financial well-being.
• Refer patients to education on healthcare delivery, reimbursement systems, prescription programs, wellness programs, community agencies, housing options, and other services.
• Use IT resources (e.g., Allscripts Care Management, EMR) to facilitate registry reporting, maintain patient populations, and improve disease outcomes through evidence-based guidelines and clinical decision support tools.
• Participate in clinical outcome measurement and identify strategies to promote population health.
• Ensure patient safety by implementing policies, procedures, and standards.
• Maintain regular, punctual attendance consistent with Orlando Health policies and federal, state, and local standards.
• Comply with all Orlando Health policies and procedures.

• Possessing knowledge and skills to provide age-appropriate care based on principles of growth and development.
• Demonstrating awareness of medical/legal issues, patient rights, and compliance with regulatory and accrediting agencies.

Requirements

• Bachelor’s degree in Social Work, Psychology, Sociology, or a related field.
• Handle with Care (HWC) Certification required for the Behavioral Health Unit.
• One (1) year of direct clinical experience with an emphasis on the population to be served in the assigned area, or a completed internship in healthcare.
