About the role
The Care Coordinator Supervisor will oversee the operations of the Health Home Servicing Children (HHSC) division. This includes monitoring the departmental phone queue, conducting new hire and ongoing training, enhancing communication and processes, and ensuring adherence to clinical standards and departmental guidelines.
Responsibilities
- Maintain quality of calls between care managers, members, and providers
- Conduct and conduct new hire and ongoing training for all clinical staff
- Support and interact with all staff in a supportive role related to care management and coordination
- Enhance communication and processes within the clinical and non-clinical areas within and between other internal operating departments
- Oversee clinical program training, retraining, and creation and updating of departmental training tools and workflows and resources
- Collaborate with providers to ensure appropriate outcomes
- Escalate crisis calls as necessary
- Log and review unacceptable inconsistencies and errors in call quality
- Perform quarterly audits of staff's chart documentation
- Support Care Management staff during management meetings and other initiatives
- Adhere to clinical standards and departmental guidelines
Requirements
- Bachelor’s degree in human services field (counseling, education, nursing, psychology, social work, etc.)
- Master’s Degree (MSW, MA, MS) preferred
- LMSW, LCSW, LMHC, LMFT, RN preferred
- Two years’ experience working with Behavioral Health and/or Substance abuse required
- Prior Supervisory Experience Required
- At least 3 years previous managed care experience, preferably in Behavioral Health/Children Social Services
- Knowledge of the Collaborative Care Model
- Experience with chronic condition management, particularly Diabetes, HIV, Heart Disease
- User/reviewer of the HCS/MAPP systems for Health Home member status preferred
- Excellent Written and Oral Communication Skills Required
