Care Coordinator RN
About the role
RN Care Coordinators work with physicians, peers, community colleagues, and others to orchestrate care across the health care continuum. They identify opportunities to continually improve patient care experiences and services, enhance population health, reduce costs, and achieve collaborative practices that exemplify Main Line Health System’s commitment to patient-centered care, community engagement, and population health.
RN Care Coordinators lead and are accountable for providing exemplary cross-continuum care coordination and care transitions. They partner with patients, families, physicians, and colleagues to develop, implement, and monitor a comprehensive plan of care, including health promotion, disease prevention, and disease management activities.
Responsibilities
- Orchestrate care across the health care continuum.
- Identify opportunities to improve patient care experiences, services, population health, and cost efficiency.
- Lead and provide cross-continuum care coordination and care transitions.
- Partner with patients, families, physicians, and colleagues to develop and monitor a comprehensive plan of care.
- Incorporate health promotion, disease prevention, and disease management activities into care plans.
Requirements
- 3 years of recent clinical experience required; leadership experience preferred.
- If ADN, 10+ years of experience required.
- Associates Degree or BSN required upon hire; if Associates, BSN required within 3 years of hire.
- Master's degree preferred.
- Active PA RN license required.
- Position requires wearing a respirator as part of the job.

Schedule
