About the role
The Care Coordinator RN ensures patients receive appropriate levels of care and transitions are timely and appropriate.
Responsibilities
- Coordinate a multidisciplinary approach to organize and coordinate effective, efficient, quality care.
- Develop a transitional plan for patients based on benefits, resources, and medical conditions.
- Use assessment, planning, implementation, coordination, monitoring, and evaluation to improve quality and cost-efficiency of care.
- Strive for improvements in the quality and cost-efficiency of medical care and the client's quality of life.
Requirements
- Education: Associate Degree or 2 years of college coursework.
- Experience: Three years of related experience. Requires working knowledge of specialized practices, equipment, and procedures.
- Licenses & Certifications: ALABAMA REGISTERED NURSE LICENSE, GEORGIA REGISTERED NURSE LICENSE.
Qualifications
- Minimum of two (2) years clinical experience, medical surgical experience preferred.
- Case management experience preferred.
- Absolutely required: Ability to function within a synergistic team concept.
- Current Georgia and Alabama RN license.
- Bachelor of Science in Nursing (BSN) or Master of Science in Nursing (MSN) preferred.
- Demonstrated ability to analyze and organize information and data.
- Demonstrated ability to organize, prioritize, and make appropriate decisions following organizational and departmental policies and procedures with minimal supervision.
- Demonstrated initiative, critical thinking, and problem-solving skills.
- Demonstrated interpersonal skills.
- Excellent verbal and written communication skills.
- Demonstrated professionalism, integrity, and ethical behavior.
- Knowledge of utilization review guidelines, case management principles, and regulatory and accreditation standards.
