Care Coordinator, Lung Cancer Screening, Pulmonology

Job Summary

Aids providers and nurse navigators with coordinating referrals and scheduling appointments for patients in the Lung Programs. Responsible for understanding insurance coverage and ensuring patient care coordination.

Essential Functions

• Completes referrals for patients and coordinates with referring and receiving practices and/or departments
• Confirms patient eligibility for lung cancer screening through tobacco intake, completes screening orders and schedules screenings
• Covers financial estimates for patients
• Obtains insurance authorizations as needed
• Directs patients to providers in their insurance plan
• Reviews benefits and educates patients on insurance coverage
• Schedules and coordinates procedures, diagnostic testing, or other services within various departments with internal and ancillary providers
• Reviews, schedules, and confirms schedules are accurate and time is properly allocated
• Follows up on all referrals to ensure no care gaps

Education/Experience

• High School Diploma or GED required
• RMA or CMA preferred due to specific responsibilities for lung programs

Skills/Qualifications

• Excellent oral and written communication skills
• Strong interpersonal skills
