Care Coordinator Assistant

About the role

Provide non-clinical administrative support to the Care Coordination process including, but not restricted to, timely discharge planning and utilization management.

Responsibilities

  • Provide department-related and other appropriate communications to internal and external customers.
  • Assist with utilization management processes to maintain compliance with federal, state and managed care guidelines.
  • Assist with transition/discharge planning and care coordination processes to implement transition plans and enhance patient/family satisfaction.
  • Maintain data integrity.
  • Provide professional, efficient and courteous service to customers while maintaining integrity in dealing with confidential issues.
  • Work effectively with peers, nursing and medical staff, maintaining a positive attitude.
  • Respond to requests personally, tactfully and courteously; obtain required information and communicate pertinent details to staff, customers and providers.
  • Relay messages to appropriate individuals efficiently and in a timely manner.
  • Promote a positive work environment and contribute to the overall team effort.

Qualifications

  • High school diploma or GED required.
  • Some college coursework preferred.
  • Two years of experience in hospital care coordination/case management, home health, quality improvement or a related field required.
