Care Coordinator Alante - PRN

Responsibilities

• Collects health information and reviews biometric data to identify gaps in care and provides disease-specific education and interventions specific to gaps identified under the guidance and direction of provider, qualified healthcare professional, and/ or registered nurse.
• Interpret policies and procedures to personnel, patients, visitors, and government agencies as required.

Qualifications

• High school diploma or GED
• CPR certified
• CNA/HCA certified
• LPN preferred
• One (1) year experience in a long-term facility preferred
