Care Coordination RN

About the Role

The Care Coordinator RN (CC RN) assesses transitional care needs, plans and facilitates patient discharges, and coordinates care across the continuum to ensure efficient and timely progression towards discharge.

Responsibilities

• Conduct psychosocial and functional assessments
• Collaborate with patients, families, physicians, and care teams
• Develop individualized discharge plans
• Identify and resolve barriers to discharge
• Facilitate post-acute referrals
• Participate in interdisciplinary rounds
• Ensure accurate documentation
• Manage care progression
• Support patient education
• Support professional development and serve as a mentor
• Ensure compliance with policies

Requirements

• Nursing degree (Associate's or Bachelor's preferred)
• Active RN license (single state or multi-state)
• BLS certification
• At least 1 year of acute care nursing experience
