Care Coordination RN

About the Role

The Care Coordinator RN (CC RN) assesses patients' transitional care needs, coordinates care across the continuum, and engages with patients and families to ensure care goals are met.

Responsibilities

• Conducts psychosocial and functional assessments
• Develops and manages discharge plans
• Collaborates with interdisciplinary teams
• Facilitates patient education
• Addresses barriers to timely discharge
• Monitors care progression and identifies delays
• Works proactively to resolve obstacles, ensuring efficient resource utilization
• Maintains accurate documentation
• Tracks avoidable days
• Participates in interdisciplinary rounds
• Supports professional development and mentors others as needed

Requirements

• RN license (single or multi-state)
• BLS certification
• At least 1 year of acute care nursing experience
• Strong communication, assessment, organizational, and problem-solving skills
• Knowledge of healthcare regulations and community resources
• Ability to demonstrate maturity, confidence, and work collaboratively in a fast-paced environment
