About the Opportunity
The Care Concierge Coordinator role involves leveraging case management skills to support members in a fast-paced call center environment. This position is responsible for managing therapy cases, advocating for members, coordinating with providers, and ensuring effective communication to facilitate improved member outcomes.
Responsibilities
- Deliver personalized support to members throughout their therapy journey.
- Help members navigate barriers and advocate for necessary care adjustments.
- Manage new and ongoing therapy cases with accurate documentation.
- Cookordinate with providers, pharmacies, and internal teams.
- Maintain therapy progress and prioritize member needs.
- Handle inbound and outbound communications and casework.
- Resolve inquiries using critical thinking to address complex issues.
- Adapt to changing member needs and anticipate challenges.
- Maintain clear communication with members, providers, and internal teams.
- Foster professional and empathetic relationships to ensure satisfaction.
Requirements
- Minimum of 3 years of healthcare experience focusing on care coordination or member advocacy.
- At least 3 years of customer service and case management experience, preferably in healthcare.
- Experience in a high-volume, fast-paced, evolving team environment.
- Strong problem-solving skills and ability to multitask.
- Passion for improving healthcare and helping people.
- Exceptional communication skills for quickly learning and articulating program benefits.
- Responsible, reliable, and detail-oriented.
- High school diploma or GED.
- Availability to work full-time between 9:00 AM - 5:30 PM ET or 11:00 AM - 8:00 PM ET.
- Ability to attend three weeks of training, including two weeks virtual and one week in-person in Hayward, CA.
- Willingness to travel onsite for training and collaboration.
- Bilingual skills are a plus.
Pay Range and Compensation Package
Base pay $24 per hour.
