About the role
Empath Hospice, a part of Empath Health, seeks a Care Center Assistant to provide communication, resource, and administrative support to the agency's interdisciplinary teams, patients, families, and other members of the community.
Responsibilities
- Perform administrative clerical duties for the team such as filing, processing/entering, and copying documents (including timesheets for HHA’s, volunteer hours in Raiser’s Edge, etc.), distributing mail, faxes, and phone messages in a timely manner, and maintaining office and team supplies.
- Avoid inaccuracies in information on patient charts.
- Avoid delays in distributing the appropriate paperwork to the different disciplines, departments, and/or community partners.
- Collaborate with Team Leadership to keep the team informed and prepared for weekly interdisciplinary team (IDT) meetings, supports, upcoming events, functions, agency meetings, and educational opportunities as well as participate in these functions as assigned.
- Track team census by monitoring all changes of status, levels of care, admissions, deaths, discharges, and transfers via electronic software; update patient care board and calculate team census in a timely manner.
- Work collaboratively with other team members to assist in coverage of patient care needs; handle phone requests calmly, effectively, and professionally while adhering to the agency's "communication practices" as directed by team leadership.
- Communicate patient equipment needs to durable medical equipment (DME) providers and maintain records of transactions and coordinate DME removal as identified.
Requirements
- High school diploma or general education degree (GED); minimum 2 years administrative experience in hospice or related field preferred.
- Excellent verbal and written communication skills.
- Solid customer service skills.
- Excellent documentation and organizational skills, including time management, multi-tasking, and prioritizing.
- Self-direction, independent judgment, discretion, and ability to work and collaborate with all team members, including volunteers.
- Strong computer knowledge in MS Word, Excel, and Outlook, and the capability to learn additional software as needed.
- Demonstrated ability to be sensitive to the needs of the staff and Empath Health clientele as well as flexibility within an organized system.
- Medical terminology background preferred.
Qualifications
- Education and/or Experience: High school diploma or general education degree (GED); minimum 2 years administrative experience in hospice or related field preferred.
Skills
- Administrative experience in hospice or related field.
- Excellent communication and customer service skills.
- Organizational and time management skills.
- Computer proficiency in MS Office.
- Medical terminology background.
