Care Advocate

About the role

We're hiring an empathetic, self-directed Certified Community Health Worker (CCHW) to support Medicare patients in a fast-paced, growing program. This role is 90% remote with 2-3 local site visits per month (expenses reimbursed).

Requirements

• Active CCHW certification (required)
• 2+ years of community health, patient advocacy, or case management experience
• Ethernet (hard-wired) internet connection required
• Quiet, private home workspace (no public spaces)
• Strong time management and attention to detail

What You'll Do

• Provide patient advocacy and resource coordination
• Connect members to community services
• Conduct occasional local outreach visits

$23/hour | Full-Time
