Cardiovascular Program Specialist
Job Summary
As our Cardiovascular Program Specialist, you will support patients in achieving optimal cardiac outcomes. You will serve as a valuable resource to staff, the multidisciplinary team, and the community, focusing on the holistic evaluation of patients with end-stage cardiovascular disease.
Responsibilities
- Coordinates the referral process and introduction of patients for evaluation and assessment.
- Receives incoming referrals from physicians, hospitals, and responds quickly to facilitate rapid access to the system.
- Contacts referring providers, requests, receives, and reviews pertinent clinical records, and coordinates record review with multi-disciplinary team members.
- Works collaboratively with the program Medical Director and multi-disciplinary team to schedule evaluations and procedures.
- Triages referrals based on written criteria to ensure patient needs are met on a priority level.
- Communicates back to referring physicians/hospitals the plans for acceptance into the program.
- Educates patients and families about the referral process, steps of evaluation, and keeps them informed throughout the entire process.
Requirements
- Required: Bachelor's of Nursing or equivalent work experience and 5 years cardiovascular experience.
- Registered Nurse: Preferred in Arizona.
- Previous experience with VAD, ECMO, CV Surgery, and cardiac intensive care, cath lab, or CTOR.

Pay Range
$45.26 - $67.32 /hour
