Captain - Mobile Integrated Health

About the role
The MIH Captain provides operational supervision, clinical coordination, quality improvement, and program oversight for Burke County Emergency Services community-based service programs, including Community Paramedicine, Mobile Integrated Health (MIH), post-overdose response initiatives, and the County's non-emergency wheelchair transportation program. The EMS Captain serves as the operational liaison between Burke County Emergency Medical Services and the Burke County Opioid Response Department to support integrated field operations, participant-centered care, and coordinated service delivery.
The position is responsible for supervising Community Paramedics, coordinating specialty transportation services, supporting field personnel, monitoring operational performance, facilitating interdisciplinary collaboration, completing employee evaluations, and assisting with the development and implementation of innovative EMS programs. Although administratively assigned to Burke County Emergency Services, the Captain works collaboratively with the Burke County Opioid Response Department, healthcare providers, behavioral health agencies, public safety partners, and community organizations to improve access to care, reduce unnecessary emergency system utilization, and enhance patient outcomes.
The position is generally assigned a Monday through Friday work schedule but maintains operational flexibility to respond to field incidents, provide supervisory support, oversee specialty operations, assist with complex participant needs, and participate in an after-hours leadership on-call rotation.
Responsibilities
- Operational Supervision
- Provide day-to-day supervision of Community Paramedics assigned to Mobile Integrated Health and community response initiatives.
- Coordinate daily staffing assignments, deployment, workload distribution, and field operations.
- Provide coaching, mentoring, and leadership to Community Paramedics.
- Conduct field observation and provide performance feedback.
- Assist with orientation, onboarding, and field training of new Community Paramedics.
- Assist EMS leadership with employee evaluations, performance improvement plans, and professional development.
- Support employee engagement, retention, teamwork, and professional growth.
- Assist with conflict resolution and personnel issues.
- Participate in recruitment and hiring activities as assigned.
- Assist with schedule development, leave management, and operational coverage.
- Participate in an established leadership on-call rotation.
- Respond to field incidents requiring supervisory assistance.
- Mobile Integrated Health & Community Paramedicine
- Coordinate Mobile Integrated Health and community paramedicine activities.
- Support post-overdose response initiatives.
- Coordinate follow-up services for individuals with substance use disorders, behavioral health conditions, chronic disease, housing instability, and other complex needs.
- Participate in participant case review and care planning.
- Assist with development of individualized care plans.
- Support transitions following emergency department visits, hospitalization, treatment admission, incarceration, or referral from community partners.
- Monitor participant engagement and referral completion.
- Coordinate services with Peer Support Specialists, healthcare providers, behavioral health agencies, and community organizations.
- Promote participant-centered, trauma-informed, and recovery-oriented care.
- Community Transportation Program Management
- Oversee daily operations of the County's non-emergency wheelchair transportation program.
- Supervise personnel assigned to specialty transportation services.
- Coordinate scheduling, staffing, and deployment of wheelchair transport resources.
- Monitor service quality, response times, and customer satisfaction.
- Develop operational procedures and workflows for non-emergency transportation.
- Ensure compliance with applicable EMS regulations, County policies, and transportation standards.
- Coordinate transportation services with hospitals, nursing facilities, dialysis centers, rehabilitation facilities, physician offices, and other healthcare providers.
- Monitor vehicle readiness, maintenance schedules, equipment inventories, and fleet utilization.
- Review transportation documentation for quality, accuracy, and compliance.
- Assist with budgeting, equipment replacement planning, and future expansion of transportation services.
- Identify opportunities to improve operational efficiency and service delivery.
- Collect and analyze utilization, financial, and performance data to support program evaluation and planning.
- Clinical Coordination
- Ensure Community Paramedics operate within approved EMS protocols and scope of practice.
- Coordinate with the EMS Medical Director regarding clinical protocols, quality concerns, and program development.
- Serve as the primary EMS liaison for community paramedicine and Mobile Integrated Health operations.
- Assist with implementation of new clinical protocols.
- Review complex participant situations.
- Support clinical decision-making when requested.
- Coordinate interdisciplinary communication regarding participant care.
- Quality Assurance & Continuous Quality Improvement
- Review documentation for completeness, quality, and compliance.
- Conduct quality assurance reviews.
- Monitor adherence to departmental policies, EMS protocols, grant requirements, and documentation standards.
- Identify training opportunities and operational improvements.
- Assist with case review and quality improvement meetings.
- Monitor operational performance metrics.
- Participate in Continuous Quality Improvement initiatives.
- Assist with data collection and reporting.
- Recommend improvements to policies, procedures, workflows, and operational practices.
- Training & Professional Development
- Coordinate orientation for new Community Paramedics.
- Develop and facilitate training programs.
- Coordinate continuing education.
- Monitor certification and credentialing requirements.
- Mentor Community Paramedics in field operations.
- Promote evidence-based community paramedicine practices.
- Coordinate interdisciplinary training opportunities with BCOR, EMS, healthcare providers, and behavioral health partners.
- Program Administration
- Assist with implementation of Mobile Integrated Health initiatives.
- Assist with implementation of grant-funded projects.
- Participate in strategic planning.
- Develop operational procedures.
- Assist with budget planning.
- Monitor equipment, vehicles, and supplies.
- Prepare reports and operational summaries.
- Attend leadership meetings and partner meetings.
- Represent EMS during collaborative initiatives.
- Provide administrative oversight for assigned specialty EMS programs.
- Coordinate development and implementation of new community-based EMS initiatives.
- Assist with strategic planning for Community Paramedicine, Mobile Integrated Health, specialty transportation, and other innovative EMS programs.
- Evaluate emerging community health needs and recommend program enhancements.
- Monitor grant-funded projects and assist with sustainability planning.
- Prepare operational reports, program evaluations, budget recommendations, and presentations for EMS leadership.
- Community Collaboration
- Maintain effective working relationships with the Burke County Opioid Response Department.
- Collaborate with Peer Support Specialists and peer leadership.
- Work closely with hospitals, behavioral health providers, treatment agencies, public health, law enforcement, and community organizations.
- Participate in multidisciplinary case conferences.
- Support community outreach and educational initiatives.
- Promote Mobile Integrated Health throughout Burke County.
Requirements
- Associate degree in Emergency Medical Services, Healthcare Administration, Public Health, Nursing, or a related field required and/or experience equivalent.
- Bachelor's degree preferred.
- Current North Carolina Paramedic credential required.
- Current North Carolina Community Paramedic credential, or ability to obtain within one year of appointment.
- Minimum three years’ experience as a licensed Paramedic.
- Minimum one year experience in Community Paramedicine, Mobile Integrated Health, Post-Overdose Response, or other field-based integrated healthcare programs.
- Demonstrated leadership, mentoring, precepting, or supervisory experience preferred.
- Experience working collaboratively with healthcare, behavioral health, and public safety partners preferred.
Special Requirements
- Current Community Paramedic credential or ability to obtain within 12 months of start date.
- Advanced Life Cardiac Life Support, Pediatric Advanced Life Support, Incident Command System (ICS) for first four course, Trauma Course.
- Valid North Carolina driver's license.
- Maintain all EMS credentials and continuing education requirements.
- Maintain affiliation with Burke County Emergency Services.
- Ability to work Monday-Friday, generally 8:00 AM-5:00 PM.
- Ability to work outside normal business hours as operational needs require.
- Participation in an after-hours leadership on-call rotation.
- Ability to respond to field incidents throughout Burke County.
- Ability to operate County vehicles and medical equipment.
