About the Role

This position coordinates the overall functions of the Patient Logistics department and Transfer Center in conjunction with the Manager. The role involves managing bed flow and transfer center functions through the capacity management board and CAD system, providing daily operational oversight of patient flow activities, and collaborating with various departments to ensure timely patient movement and customer satisfaction.

Responsibilities

  • Coordinates bed flow and transfer center functions through management of the capacity management board and CAD system.
  • Provides daily operational oversight and decision-making for patient flow activities by collaborating with Patient Access, Emergency and Surgical Services departments, physician offices, and others to assess, plan, and implement patient flow activities.
  • Communicates with clinical unit staff regarding transfers and admissions, facilitating timely patient movement.
  • Collaborates with physicians, care managers, and floor staff to ensure timely coordination of discharge and transfer.
  • Assesses patient status/acuity to prioritize inpatient admissions, critical care admissions, and internal/external transfers for efficient patient movement.
  • Serves as an expert clinical resource for patient placement, ensuring smooth functioning of the Logistics Center process to reduce wait times and avoid unnecessary patient movements.
  • Ensures throughput on units and compliance with the transfer process through management of people and processes.
  • Identifies and anticipates barriers to discharge/patient flow activities and works to ensure throughput in ED, inpatient units, and compliance with the transfer process.
  • Activates the Hospital Surge/Capacity plan based on established guidelines and manages identified overflow patient care areas.
  • Reviews blocked and closed beds daily to complete the daily closed bed report with bed numbers and reasons for closure.
  • Monitors inpatient bed status and upgrades bed cleaning status via the bed tracking system to expedite patient flow.
  • Coordinates with Transport and Environmental Services supervisors to expedite patient flow.
  • Monitors patient movement within and outside Capital Health to ensure timely and efficient transfers, avoiding unnecessary length of stay.
  • Coordinates the transport of patients to and from Capital Health facilities and transfers requiring emergent or non-emergent Neurological, Trauma, Cardiac, or other specialized medical services.
  • Facilitates physician-to-physician communications for admissions to Capital Health facilities or transfers out of Capital Health.
  • Dispatches and tracks Wheelchair, BLS, and SCT services using computer-aided dispatch (CAD) or outsources requests to contracted agencies.
  • Requests Uber transportation for discharged patients.
  • Coordinates with the County 9-1-1 Communications Center and Capital Health EMS units or supervisors.
  • Receives updates from Capital Health facilities reporting diversion status and reports the status to various County and State agencies.
  • Communicates openly and effectively, providing ongoing education to the healthcare team on patient flow strategy and rationale for patient placement.
  • Promotes team momentum, enthusiasm, and pride through effective communication styles.
  • Functions as a department team member to support Capital Health’s goals, objectives, and established policies.
  • Performs other duties as assigned.

Requirements

  • Associate's degree and graduation from an accredited school of nursing.
  • Four years of experience in an acute care hospital setting, including minimum Telemetry experience (Critical Care experience preferred).
  • AHA BLS - Healthcare Provider certification.
  • Registered Nurse (RN) licensure in New Jersey.

Schedule

Shift: 6:00 PM - 6:30 AM (rotating weekends and holidays)
Scheduled Weekly Hours: 36

Pay

Pay Range: $41.60 - $54.63 per hour (reflects compensation for a full-time equivalent (1.0 FTE) position; actual compensation may differ based on assigned hours and position status).
