CAP Case Manager / PT / Iredell & Surrounding Area

About the role

We are hiring a CAP Case Manager in the Iredell and surrounding area. This role involves providing basic case management services to medically fragile children who are at risk of institutionalization and ensuring their health, safety, and well-being are maintained in their primary residence. The position requires coordination with an RN to ensure quality care.

Responsibilities

• Provide case management services to medically fragile children
• Ensure quality care through coordination with RNs
• Coordinate services to prevent institutionalization
• Maintain health, safety, and well-being in primary residences

Requirements

• Positive and personable demeanor
• Ability to make a real difference in the lives of those with intellectual, developmental disabilities, and mental health/substance use challenges
• Experience with case management services
• Pre-employment screening including criminal background check, OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities
• Proof of employment history or references (if required)
• Driving requirement: proof of driver's license, insurance, and vehicle (if required)
