About the role
As a collaborative member of the Cancer Center Clinics team, the Cancer Center Patient Coordinator plays an enhanced role in providing support for patients by engaging in patient education, preventive health, and chronic disease management. This role involves reinforcing the clinical plan of care and working closely with a multidisciplinary care team to ensure continuity of care. The CC Patient Coordinator is responsible for facilitating communication between patients, their families, and healthcare providers, while ensuring the seamless coordination of clinic schedules, tests, and treatments. This position requires excellent organizational skills, attention to detail, and compassion.

Experience
- A minimum of 2 years of related experience in Oncology is required
- A minimum of 2 years of experience in Oncology care coordination is preferred
- Oncology Care Coordination includes, but is not limited to: supporting patients between care settings (inpatient to outpatient) and subsequent services needed (e.g. rehab, infusion, other); scheduling of appointments that support the diagnosis, workup, and treatment of patients; and communicating with patients and families as it directly impacts their care
Education
- Associate degree is required. In lieu of degree, a minimum of 2 additional years of related experience in Oncology care coordination is required
- Bachelor's Degree is preferred
Training
- Completion of a Medical Terminology Course within 90 days of hire is required
Skills
- Must have excellent computer skills; proficiency in Microsoft Office Suite, knowledge of medical terminology, typing and office equipment is required
- Experience working with Electronic Medical Records (EMR) is required
- Professional written/verbal communication and interpersonal skills
- Ability to identify critical issues and exercise independent judgment
- Demonstrated detail orientation with the ability to analyze, problem solve, organize and manage multiple priorities
- Excellent follow up skills
- Proficiency in Word and Excel programs, Microsoft Office systems
- Critical thinking skills
- Excellent prioritization, organization, and multi-tasking skills
- Ability to work independently (self-directed) and have excellent follow through
- Must have excellent communication and customer service skills
- EPIC Chart Review experience preferred with the understanding of health care facilities and processes
