Cal-AIM Housing Case Manager
SUMMARY
The Cal-AIM Housing Case Manager, under the supervision of the Program Supervisor, works with community service providers to build relationships with the community, provide advocacy, and link to benefits and housing. The primary goal is to identify unsheltered homeless and other individuals through community referrals from homeless service providers and social service agencies. In identifying highly vulnerable homeless, unsheltered, and other individuals, the Case Manager supports Medi-Cal members experiencing homelessness or those at risk of homelessness, helping them navigate housing resources through Community Supports services provided by Medi-Cal Managed Care Plans (MCPs). The role is vital in addressing health-related social needs to promote healthier living and prevent more costly healthcare interventions.
Responsibilities
- Build relationships with community service providers and individuals experiencing homelessness or at risk of homelessness
- Provide advocacy and link individuals to necessary benefits and housing resources
- Navigate housing resources for Medi-Cal members experiencing homelessness or at risk of homelessness
- Support identification of unsheltered homeless and other individuals through community referrals
- Work with Community Supports services provided by Medi-Cal Managed Care Plans (MCPs)
- Address health-related social needs to promote healthier living and prevent more costly healthcare interventions
Requirements
- Bachelor's degree in Social Work, Public Health, or related field
- Experience working with homeless populations and/or community outreach preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Proficiency in Microsoft Office Suite
Qualifications
- Current California Driver’s License
- Valid California Class C Driver’s License
- Passing score on a background check

Pay
$25.00 - $30.00 per hour based on experience and qualifications
