Bridges Liaison
About the role
We are seeking a dedicated and passionate Bridges Liaison to join our dynamic team. The ideal candidate will be a proactive leader with strong communication and problem-solving skills, committed to helping young adults achieve independence and success.
Responsibilities
- Coordinate intake, eligibility determination, enrollment, and ongoing participation in the Bridges Program
- Develop collaborative relationships with young adults based on trust, accountability, and person-centered planning
- Complete comprehensive assessments and develop individualized case plans that support independent living goals
- Carefully document all required assessments, eligibility documentation, case plans, VPA documentation, tier determinations, and other program-specific documentation
- Maintain compliance with all agency, state, licensing, and funding requirements
- Participate in rotating on-call responsibilities and respond appropriately to crisis situations
- Develop and maintain productive relationships with various community partners and internal departments
- Participate in team meetings, supervision, required training, and quality improvement activities
- Contribute to continuous improvement initiatives that strengthen program quality and client outcomes
Requirements
- Minimum of 21 years of age
- Bachelor’s degree in a human services-related field and a minimum of 2 years’ experience working with young adults with emotional or behavioral problems
- Valid Driver’s License
- Limited Liability Auto Insurance Coverage of 100/300/100
- Organizational and written/oral communications skills
- Ability to navigate and execute software (Microsoft offices, databases, electronic health records, dashboards, etc.)
- Successful completion of all required criminal background checks and drug screenings
- Ability to lift and carry supplies and/or equipment, up to 50 lbs.
- Ability to adhere to scheduled and unplanned deadlines
- Willingness to travel and attend training
- Successful completion of all required training pertaining to job

Pay
The salary range for this position is $40,000 - $60,000 annually, depending on experience and qualifications.
Schedule
This is a full-time position with flexible hours to accommodate the needs of the young adults served by the Bridges Program.
