Responsibilities

• Provides case management to emancipated foster youth in Northeast Ohio.
• Refers and links Young Adults in the area they reside.
• Coaches and educates young adults with Independent Living Skills and Transitional Youth skills.
• Works within the community, meeting young adults in their home environment.
• Assists with locating and securing housing.
• Educates on budgeting.
• Links to community resources.
• Completes paperwork in a timely manner.

Requirements

• Minimum of a bachelor’s degree in social work, Human Services, Behavioral Health, or a related field.
• Two years of experience working with families and/or children in the child welfare system (preferred).
• Shall be an employee of the Bridges provider agency.
• Possess a valid driver’s license.
• Be twenty‑one (21) years of age or older.

• Knowledge of SACWIS (a plus)
