About the role
This role is for a registered nurse (RN) specializing in patient navigation to enhance patient experience and clinical outcomes.
Responsibilities
- Collaborate with physicians and interdisciplinary teams to triage, coordinate, and manage patient care.
- Educate patients and serve as a communication point for families.
- Develop and implement navigation activities.
- Promote shared decision‑making.
- Address health literacy and communication needs to support patient engagement.
Requirements
- Five years of RN navigator work (preferred).
Qualifications
- Graduation from an accredited nursing program.
- California RN licensure.
- BLS certification.

Schedule
Full‑time, day shifts, non‑exempt status, 40 hours per week.
