About the role
The role involves coordination and implementation of strategies to ensure quality, efficacy, and cost-effectiveness for oncology patients in various settings. The PONN also provides patient and family education, collaborates with multidisciplinary teams, and develops programs to meet community needs.
Responsibilities
- Assesses patient needs
- Orients patients to the hospital and outpatient settings
- Delivers cancer care across the continuum, from diagnosis through treatment and survivorship
- Uses the nursing process to provide patient and family education and resources
- Collaborates with the treating physician and multidisciplinary team
- Communicates and collaborates with the multidisciplinary team
- Reviews distress screening for referrals
- Communicates and collaborates with the treating physician
- Develops and reports outcome metrics to drive program development
- Assesses and implements activities that support COG standards
- Strategizes activities that support community needs assessment
- Provides emotional support for patients
- Advocates on behalf of the patient
- Empowers the patient to self-advocate
- Maintains own knowledge, ability, skills, and competencies
- Role models organizational behaviors of excellence
Requirements
- Bachelor of Science in Nursing (BSN)
- Typically requires 5+ years of experience in clinical specialty (Oncology)
- Current Illinois RN license
- CPR certification
- OCN Certification or Nurse Navigator Certification obtained within 2 years of hire
