Blended Case Mgr
Essential Position Responsibilities
- Coordination of meetings and necessary evaluations for participants
- Cookordination of access to medical and/or other treatment services as needed by participants
- Responsible for participant/family outreach in matters surrounding the coordination of services
- Responsible to assist participant and families in accessing community support and resources
- Responsible to advocate on behalf of participant’s needs
- Responsible to assure ongoing communication and integration of services among all service providers and support systems involved with the child, adolescent, adult, and family
- Responsible to monitor the implementation of clinical strategies as developed with team
Qualifications
- Minimum Qualifications: Bachelor’s Degree in a mental health discipline or related human services field AND 2 years’ experience providing Targeted Case Management services in Philadelphia OR Master’s degree in human services, psychology or Social Work
- Experience working adults with serious mental health AND substance use disorders
- Valid Driver’s License (issued by state in which employee lives), safe driving record and proof of insurance coverage
- Experience navigating multi-system services and a working knowledge of available community resources
- Demonstrate a genuine respect for all adults and families
- Demonstrate an understanding of cultural competence and the principles of recovery
- Adult and Pediatric CPR Certification Required

The Northeast Treatment Center founded in 1970 is one of the largest behavioral health and social service agencies in the Philadelphia Region. Services are offered at 15 sites throughout Greater Philadelphia, the Lehigh Valley and the state of Delaware. NET Centers (NET) offers support and hope to individuals and families at times of great stress in their lives. A nonprofit agency, NET provides a continuum of trauma informed behavioral health and social services designed to meet complex needs of all family members. These include mental health, addiction treatment, foster care, residential group care, adoption, juvenile justice services and child welfare services. While some services address the needs of the family as a whole and some target individuals, we always strive to foster positive connections within the family to help all family members heal, recover and rebuild.
