Job Description
The Case Manager position supports the physician, primary medical homes, and interdisciplinary teams in facilitating patient care, enhancing clinical outcomes, and improving patient satisfaction while managing the cost of care and providing timely and accurate information to payors.
- Facilitates patient care progression throughout the continuum.
- Collaborates with the physician and multidisciplinary team to facilitate care for designated case load.
- Maintains active communication with physicians, nursing, and other members of the care team to ensure timely, appropriate patient care.
- Solves system problems impeding diagnostic or treatment progress.
- Resolves delays and obstacles to discharge.
- Identifies and resolves issues proactively.
- Coordinates discharge planning.
- Develops and implements clinical pathways and other Case management tools.
- Monitors LOS and ancillary resource use.
- Communicates with Resource Center for covered day reimbursement certification.
- Ensures safe care to patients, staff, and visitors.
- Promotes individual professional growth and development.
- Models Memorial Hermann's service standards.
Principal Accountabilities
- Facilitates patient care progression throughout the continuum.
- Works collaboratively and maintains active communication with physicians, nursing, and other members of the care team to effect timely, appropriate patient care.
- Addresses/resolves system problems impeding diagnostic or treatment progress.
- Proactively identifies and resolves delays and obstacles to discharge.
- Utilizes advanced conflict resolution skills as necessary to ensure timely resolution of issues.
- Collaborates with the physician and all members of the multidisciplinary team to facilitate care for designated case load.
- Maintains a designated patient caseload and plans effectively to meet patient needs across the continuum.
- Provides family support, manages the length of stay, and promotes efficient utilization of resources.
