POSITION DETAILS / ESSENTIAL FUNCTIONS
- Aid individuals with mental illness, including children with serious mental illness or emotional disorders, in accessing necessary services.
- Assist in obtaining and maintaining basic living needs and skills such as housing, food, medical care, recreation, education, and employment for the consumer or their family.
- Provide services within the cultural context of the consumer and their family.
- Implement case management services based on a written, individualized service plan that assesses strengths, needs, interests, and is goal-oriented.
- Offer services in the consumer's preferred location or in the Case Management office if off-site interventions are not suitable.
- Contact the consumer or their family based on their assessed needs, ranging from weekly to monthly visits.
- Ensure immediate follow-up for consumers or families who cannot be reached during visits, continuing until their status is clarified.
- Provide crisis intervention services to children and adolescents under Case Management.
- Document the purpose of each contact and the goals and objectives of the service plan in written case notes.
- Complete and submit required paperwork by the stated deadlines.
- Maintain and ensure all documentation is included in the consumer's records.
- Provide continuity of care for hospitalized children and adolescents receiving Case Management services.
- Offer telehealth services as necessary to the consumer and their family or relevant parties as part of the consumer's case.
