Responsibilities

  • Perform all duties per the client’s individualized care plan as directed by the In-Home Care Supervisor or Manager.
  • Absorb and assist with all activities of daily living (ADL) as directed by the care plan, including chore services, medication assistance, personal care, meal preparation, shopping, and transportation.
  • Communicate with the care management team about changes in the client’s condition, requests for additional services, or changes in the care plan.
  • Provide emotional and social support to clients as needed.
  • Ensure the client’s environment is safe, clean, and orderly.
  • Transport clients to and from appointments, errands, and activities as directed.
  • Assist clients with mobility and transfers, including ambulation and transfers.
  • Maintain accurate records of client observations, changes in health or emotional condition, and medication distribution.
  • Report all changes in condition to the Client Care Manager and care management team immediately.
  • Perform general housekeeping duties and adhere to safety protocols in emergency situations.
  • Assist clients with physical therapy activities as directed by the plan of care.
  • Respond promptly and positively to client requests for assistance.
  • Conduct oneself professionally and maintain confidentiality with sensitive information.
  • Accurately record all time worked for scheduled shifts using telephony or Clear Care phone applications.
  • Represent Fedelta Home Care in a professional manner while interacting with clients, families, and team members.

Requirements

  • Current Washington State CNA, HCA, HHA, or NAR credential in good standing, or the ability to obtain certification within timelines established by Washington State regulations.
  • Nurse Delegation certification is strongly preferred but not required.
  • A valid Washington State driver’s license, acceptable driving record, and proof of current auto insurance.
  • Demonstrated experience and tact in communicating with diverse populations.
  • Strong time management, interpersonal, patience, listening, and knowledge of generally accepted care standards.
  • Effective verbal and written communication skills, including the ability to communicate through various channels including in-person, phone, email, and text messages.
  • Experience in caregiving, especially if familial.
  • Computer skills: proficiency in Microsoft Office Suite, Word, Excel, and Outlook; experience with health care programs such as WellSky Personal Care.
  • The ability to lift and move up to 50 pounds occasionally and more than 100 pounds assisted when providing direct patient care.
  • The ability to perform activities such as preparing and analyzing data, viewing computer terminals, reading, operating office equipment, operating a motor vehicle, and determining accuracy, neatness, and thoroughness of assigned work.

Qualifications

  • Minimum of six months industry-related experience.
  • Education: Requires Certified Nursing Assistant (C-NA), Home Care Aide (HCA), Home Health Aide (HHA), or Nursing Assistant Registered (NAR).
