Responsibilities

  • Perform all duties per the client’s individualized care plan as directed by the In-Home Care Supervisor or Manager.
  • Absorb and assist with all activities of daily living (ADL) as directed by the care plan, including chore services, medication assistance, personal care, meal preparation, shopping, and transportation.
  • Communicate with the care management team about changes in the client’s condition, requests for scope of care outside the individualized care plan, or requests for services outside the scope of license or credentials.
  • Provide emotional and social support as required by the client’s preference or as directed by the individualized care plan.
  • Engage clients in a manner that promotes their independence and maintains their dignity to perform necessary tasks and activities of daily living (ADL).
  • Maintain accurate client records in a timely manner and in accordance with company policy.
  • Report all changes in condition to the Client Care Manager and care management team immediately.
  • Maintain a safe, clean, and orderly environment for the client.
  • Safely transport clients to and from appointments, errands, activities, etc., as directed by the plan of care.
  • Assist clients with ambulation and mobility by safely assisting with mobility and transfers of clients, including assisting clients in and out of bed, chairs, wheelchairs, standby assistance, etc.
  • Adhere to proper safety protocols and company procedures in emergency situations.
  • Assist clients with physical therapy activities as directed by the plan of care.
  • Respond promptly and positively to client requests for assistance.
  • Provide excellent hands-on care and bedside manner with patients.
  • Regularly and constantly report to all scheduled shifts.
  • Accurately record all time worked for scheduled shifts via Telephony or Clear Care phone applications.
  • Cultivate a professional and knowledgeable presence while representing Fedelta Home Care to clients.
  • Maintain strict confidentiality with sensitive information and proprietary company information.
  • Conduct oneself professionally and communicate with team members, clients, and families while engaged in work responsibilities for Fedelta Home Care.

Requirements

  • Current Washington State CNA, HCA, HHA, or NAR credential in good standing is required, or the ability to obtain required certification within the timelines established by Washington State regulations.
  • RN Delegation certification is strongly preferred, but not required.
  • A valid Washington State driver’s license, acceptable driving record, and proof of current auto insurance are required for employees who operate a vehicle while performing duties on behalf of Fedelta Home Care.
  • Candidates must also be able to successfully complete all pre-employment and ongoing background check requirements in accordance with state regulations and company policy.
  • Demonstrated experience in Microsoft Office Suite, Word, Excel, and Outlook.
  • Experience with or the technical aptitude to proficiently use health care programs such as WellSky Personal Care.
  • The physical demands of this job are moderate, requiring the ability to lift up to 20 pounds frequently, up to 50 pounds occasionally, and up to 100 pounds assisted occasionally.

Qualifications

  • Already fully licensed as a CNA/HCA OR have completed 70 hours of Basic Training and are enrolled in a paid HCA Training program.
  • Nurse Delegation certification is a plus.
  • Reliable with vulnerable clients.
  • Good communication skills via phone call, email, and/or text message.
  • Comfortable driving clients for essential appointments and errands - mileage reimbursement.
  • Experience in caregiving preferred - familial experience counts!
