About the role

The Care Coach evaluates members' needs and requirements to achieve and/or maintain optimal wellness by guiding members and families toward and facilitating interaction with appropriate resources for care and well-being. The role involves straightforward to moderately complex assignments, employing various strategies and techniques to support members' wellness through service coordination and resource management. Responsibilities include identifying and resolving barriers to effective care, monitoring patient progress, and ensuring desired outcomes through continuous assessment and active care planning.

Work is managed and guided by precedent, documented procedures, regulations, and professional standards, with some interpretation required. The role follows established guidelines and procedures.

Responsibilities

  • Evaluate members' needs to achieve or maintain optimal wellness.
  • Guide members and families toward appropriate resources for care and well-being.
  • Coordinate services and resources to support members' wellness.
  • Identify and resolve barriers that hinder effective care.
  • Monitor patient progress toward desired outcomes using assessments, data, and member conversations.
  • Make decisions regarding work approach and priorities while following direction.
  • Adhere to established guidelines, procedures, and professional standards.

Requirements

  • Bachelor's degree in a health and human services field.
  • 2 or more years of related experience.
  • Proficiency in using a variety of electronic information applications and software programs.
  • Bilingual (preferred but not required).

Preferred Qualifications

  • Prior experience with Medicare and Medicaid recipients.
  • Experience with electronic case note documentation and multiple computer applications/systems.
  • Experience with health promotion, coaching, and wellness.
  • Knowledge of community health, social service agencies, and additional community resources.

Work at Home Requirements

  • Self-provided internet service must meet the following criteria: minimum download speed of 25 Mbps and upload speed of 10 Mbps; wireless, wired cable, or DSL connection suggested.
  • Work from a dedicated space free of ongoing interruptions to protect member PHI/HIPAA information.
  • Occasional travel to Humana's offices for training or meetings may be required.

Scheduled weekly hours: 40

Pay

The starting base pay range for this full-time (40 hours per week) position is $53,700 - $72,600 per year. Pay may vary based on geographic location and individual qualifications, including skills, knowledge, experience, education, and certifications.
