About the role

Bilingual Health Coaches are needed to provide health education and support services to patients in a remote setting. The role involves conducting health assessments, providing personalized health plans, and facilitating patient engagement.

Responsibilities

• Conduct health assessments and develop personalized health plans
• Provide ongoing health education and support services
• Facilitate patient engagement and satisfaction

Requirements

• Bilingual fluency in English and Spanish
• Experience in health coaching or related field
• Strong interpersonal and communication skills
• Ability to work independently and manage time effectively

Qualifications

• High school diploma or equivalent
• Some college coursework or relevant experience
• Valid driver’s license and reliable transportation (for travel)
