Job Summary

We are looking for experienced Bilingual (Hmong/English) family and/or adult staff providers, MN, DO, PA or NP.

Responsibilities

  • Serve as a point of contact for patients, families, vendors, co-workers, etc., through respectful, courteous, and culturally sensitive interactions.
  • Participate actively in team efforts within and across departments to support the organization's goals.
  • Assist in areas outside of primary responsibilities for the good of the organization.
  • Demonstrate effective, culturally sensitive communication skills and communicate verbally and in writing with a variety of people.
  • Adhere to organizational policies related to patient's rights for confidential care.
  • Create and manage appropriate patient care plans, including adequate diagnosis, treatments, consultations, and outcomes.
  • Affirm patients' continuity of care by scheduling return visits, completing treatment plans, and addressing chronic issues, tests, and no-shows.
  • Provide interpersonal care that includes introducing oneself, treating patients with care, compassion, and courtesy, listening to and answering questions from patients, and clearly explaining diagnoses and treatments.
  • Offer whole-person care by referring patients to social workers, psychologists, psychiatrists, and integrating social and structural determinants of health, including historical trauma, structural racism, and biological, social, psychological, and environmental factors.

Work Environment and Physical Demands

This job operates in a healthcare setting. This role may occasionally come into contact with patients who may have contagious illnesses. This position is very active and requires standing, walking, bending, kneeling, and stooping. The employee may need to lift and/or move items over 20 pounds.
