About the role

GGRC is committed to assisting people with intellectual and developmental disabilities lead lives of liberty and opportunity. This role offers a hybrid schedule, allowing you to work within homes of clients in San Francisco County, our San Francisco office, and from your own home.

Responsibilities

  • Work with individuals with developmental disabilities over the age of 3 years old and their families to advocate for and secure needed services.
  • Assess individuals supported and their needs, including those of their families, and develop, implement, and coordinate their Individual Program Plan.
  • Manage a wide range of active cases requiring professionally trained social work skills, including creative planning and acquiring services.
  • Participate in ongoing social assessments and overall case planning for persons with developmental disabilities.
  • Obtain or collect information or data for use by an interdisciplinary team.
  • Design, monitor, and revise program plans; analyze data and identify barriers to individuals' developmental objectives.
  • Initiate and terminate regional center funding and ensure purchased services are provided.
  • Prepare required reports and documentation in a timely and accurate manner; maintain records in compliance with federal and state requirements.
  • Act as an advocate for individuals supported and their families to assure receipt of entitled services.
  • Encourage individuals and families to fully participate in coordinating and utilizing available resources.
  • Review and ensure individuals receive appropriate and effective services from existing community resources.
  • Initiate referrals and monitor progress of vendored services; evaluate availability and effectiveness of services.
  • Identify community service needs or gaps and assist in developing needed services and improving quality.
  • Provide outreach services as assigned.
  • Participate in internal and community committees and task forces as assigned.

Requirements

  • Bachelor's Degree from an accredited school of social welfare or related human services field; Master's Degree preferred.
  • Fluency in Spanish, including thorough knowledge of colloquial and relevant technical/functional terminology.
  • Knowledge of principles, objectives, and techniques of counseling individuals; normal and abnormal individual and group behavior; human growth and development of family relationships.
  • Knowledge of community structure and organization and how to intervene constructively on a client's behalf.
  • Case management experience is strongly desired.
  • Ability to work independently, exercise judgment in decision-making, and cooperate as part of an interdisciplinary team.
  • Excellent communication skills for producing business documents and conveying complex information clearly and professionally.
  • Ability to stimulate cooperation from clients, families, and community agencies or individuals.
  • Ability to establish and maintain effective working relationships.
  • Access to reliable transportation for visiting programs often without advanced notice.
