Bilingual RN Care Coach - Bonus Eligible
About the role
CircleLink Health is seeking a passionate, tech-savvy Registered Nurse to work remotely and manage patients enrolled in Medicare's Chronic Care Management Program. This part-time role involves 20-25 hours per week, where an RN Care Coach will call patients monthly to provide education, coordinate care, and coach on self-management strategies.
Responsibilities
- Utilize specialized care management software to call Medicare patients with 2 or more chronic conditions.
- Build and maintain rapport with patients to help them achieve improved health through SMART goals and education on self-management strategies.
- Implement and improve the Plan of Care by updating medications, appointments, biometrics, symptoms, and interventions.
- Connect patients with community resources such as transportation, personal care needs, prescription/DME assistance, social services, etc.
- Conduct Transitional Care Management activities for high-risk patients discharged from the hospital or ER to reduce unnecessary readmissions.
- Close care gaps by encouraging and assisting with preventive care measures, such as annual well visits, vaccines, cancer screens, follow-up/specialist appointments, etc.
Requirements
- Fluent in English and Spanish
- Self-directed, able to work independently with little supervision while meeting performance metrics
- Passion for nursing and improving patient outcomes
- Good with technology and eager to learn and use new software
- Excellent organizational and time management skills
- Timely communication is essential, and nurses are expected to respond to all messages and emails within 24-48 hours
- Strong critical thinking and problem-solving skills
Qualifications
- Current, unrestricted Multistate RN or New Mexico RN license
- Proficiency with EPIC electronic health record and web-based applications (preferred)
- Case Management or Chronic Disease Management experience (preferred)
- Certified Diabetes Educator (preferred)
- Experience with Motivational Interviewing or other behavior change communication techniques (preferred)

Benefits
- Compensation: $15.00 for the first 20-minute increment, $12 for the second 20-minute increment, and $11.50 for the third 20-minute increment.
- Bonus eligibility: Three months of employment and meeting all performance expectations qualify for a $300 performance bonus.
- Pay timing: Monthly via direct deposit, 40 days after the last day of the first month of service. Subsequent payments are approximately every 30 days after each month.
Schedule
RN Care Coach must have a strong internet-connected computer and be available for 20 hours per week between 8 am and 5 pm MST, Monday-Saturday. The schedule is flexible, but not every day is required.
